By submitting comments, you give consent to allow the National Health Council (NHC) and the Campaign to Put Patient First to use your name, photo, video, and comments ("Your Story") in its efforts to secure better health care for all Americans. You understand that your information may be used for marketing, publicizing, or otherwise promoting the National Health Council's mission to improve health care, and that your information may be disseminated in any medium without geographic or time restriction.

You waive and release NHC from any claims that you may have against NHC or its officers, directors, employees, agents, or affiliates arising out of its use or dissemination of your information. You also waive any right to royalties or other compensation for NHC's use and dissemination of your information. You assign all rights, including copyright, to your information to NHC. You represent and warrant that you have the authority to provide your information to NHC and that no one else owns or has superior rights to this information. You have read, fully understand, and voluntarily agree to be legally bound by this waiver, release, and authorization form.
